Metabolic & Pre-diabetes
- HbA1c β every 3 months while in pre-diabetes range
- Fasting insulin + glucose β HOMA-IR (insulin resistance score)
- Fasting triglycerides + HDL (metabolic syndrome markers)
- Uric acid β elevated in stone and pre-diabetes
MTHFR / Methylation
- Homocysteine β target <8 Β΅mol/L (ideally <7)
- RBC folate (not serum) β better marker of tissue stores
- Serum B12 + MMA (methylmalonic acid) if B12 is borderline
Celiac Recovery
- tTG-IgA β monitors gut healing (should normalise 12β18 mo strict GF)
- Ferritin (not just hemoglobin)
- 25-OH Vitamin D
- Zinc, RBC magnesium
Kidney Stone
- 24-hour urine collection (gold standard): oxalate, citrate, calcium, uric acid, volume, pH
- Serum calcium + PTH β rule out hyperparathyroidism
- Urinalysis with microscopy β monitor for crystals / infection
Klebsiella / Gut
- Comprehensive stool panel (GI Effects, Doctorβs Data, or similar)
- Urine culture if any UTI symptoms β Klebsiella requires targeted antibiotics
- Consider urine oxalate in 24-hour panel to assess Oxalobacter gap
APOE4 / Cognitive
- Lipid panel with LDL particle size (NMR preferred for APOE4)
- hsCRP (high-sensitivity C-reactive protein) β inflammation marker
- APOE genotyping (if not already done β confirms e3/e4 vs e4/e4)
- Baseline cognitive screening (MoCA) β establishes baseline now at 62
- β pTau217 / Ξ²-Amyloid 1-42 Plasma Ratio (Lumipulse G) β research study, blood draw May 19 β detects amyloid plaque accumulation via blood draw (~95% concordant with PET scan); critical early marker for APOE4 carriers
